What is a Hydrafacial?

“Hydrafacial” is commonly used to describe a hydradermabrasion-style facial procedure involving several steps, which may include:

  • Cleansing
  • Controlled exfoliation
  • Suction-assisted extraction
  • Application of hydrating solutions
  • Application of selected serums
  • Optional supportive steps according to the skin concern

Hydradermabrasion combines crystal-free surface exfoliation with pneumatic or suction-assisted application of skincare solutions. A small controlled study reported improvements in selected measures of skin quality after a course of treatment, although only 20 women participated.

Device details and regulatory claims depend on the exact system used and are discussed during consultation.

What is a medifacial?

A medifacial, or medical facial, is a broad term for a customised clinic-based facial planned according to the patient’s skin type and concern.

Depending on assessment, it may include a combination of:

  • Gentle cleansing
  • Enzyme or superficial exfoliation
  • Comedone care
  • Hydradermabrasion
  • Controlled suction
  • Hydrating masks
  • Suitable active serums
  • LED treatment where clinically appropriate
  • Moisturiser and sunscreen

There is no single universal medifacial protocol. A facial for dry, sensitive skin should not be identical to one designed for oily or congested skin.

How is a medifacial different from a salon facial?

A dermatologist-led medifacial begins with assessment of the skin rather than selection from a standard beauty menu.

The consultation may consider:

The aim is to avoid ingredients or steps that may aggravate an existing skin disorder. A facial should not be described as “medical” merely because stronger products are used.

Which skin concerns may benefit?

A customised facial may be considered for selected concerns such as:

  • Dry or dehydrated-looking skin
  • Dull complexion
  • Mild surface roughness
  • Oily skin
  • Congested pores
  • Blackheads
  • Mild comedonal acne
  • Temporary loss of radiance
  • Uneven-looking texture
  • Preparation for a special event
  • Maintenance between other dermatology procedures

The AAD states that controlled microdermabrasion can help selected patients with dullness, uneven texture, superficial dark marks and a less radiant complexion.

A facial cannot diagnose or permanently correct every concern in this list.

What does current research show?

Evidence for hydradermabrasion is more limited than the evidence available for many established medical treatments.

Published studies include:

A 20-participant trial reporting changes in fine lines, pore appearance, pigmentation and microscopic skin features after six hydradermabrasion sessions

An imaging study involving only eight volunteers

A 20-patient, open-label study of a branded clarifying protocol for mild-to-moderate acne

The acne study had no blinded control group, and an author was affiliated with the device company. These limitations mean results should be interpreted cautiously rather than promoted as proof that the treatment cures acne or permanently changes the skin.

Can Hydrafacial help acne?

A hydradermabrasion or clarifying facial may support selected patients with:

A small open-label study involving 20 adults with mild-to-moderate acne reported improvement after six treatments over 12 weeks. However, the study was small, unblinded and included company involvement, so the procedure should be viewed as supportive care rather than a replacement for established acne treatment.

Inflammatory, nodular, cystic or scarring acne usually requires a dermatologist-directed medical plan.

Can it remove blackheads?

Suction and controlled exfoliation may temporarily clear some superficial pore contents and improve the appearance of congestion.

However:

  • Not every dark pore is a blackhead
  • Deep comedones may need separate extraction
  • Aggressive suction can irritate sensitive skin
  • Blackheads can return
  • Ongoing acne care may still be required

Repeated strong extraction should not be performed over inflamed acne, broken skin or fragile blood vessels.

Can it improve dry or dehydrated-looking skin?

Selected hydrating solutions and moisturising steps may improve the temporary appearance and feel of dry or dehydrated skin.

Patients may notice:

  • Softer-feeling skin
  • Temporary plumping
  • Smoother surface texture
  • Improved visible radiance
  • Better comfort after moisturising

Small studies of hydradermabrasion and serum-assisted microdermabrasion have reported short-term improvements in hydration and skin appearance, but protocols, products and funding sources vary.

Persistent dryness may instead result from eczema, irritation, over-exfoliation, medicines or an unsuitable skincare routine.

Can Hydrafacial treat pigmentation or melasma?

A superficial facial may temporarily improve overall brightness or help selected superficial dark marks appear less prominent.

It is not a stand-alone cure for:

  • Melasma
  • Deep pigmentation
  • Hormonal pigmentation
  • Post-inflammatory hyperpigmentation
  • Freckles
  • Sun-induced lentigines

AAD guidance notes that controlled microdermabrasion may support treatment of uneven tone, superficial dark marks and melasma in selected patients, generally as one part of a broader plan.

Melasma usually requires consistent sunscreen, suitable prescription skincare and long-term maintenance.

Can a facial remove acne scars?

No routine Hydrafacial or medifacial can remove deep acne scars.

It may temporarily improve:

  • Surface smoothness
  • Dryness around scars
  • Congestion
  • Post-acne dullness
  • Superficial dark marks

Rolling, boxcar, ice-pick and tethered scars may require procedures such as:

Can it reduce pores permanently?

No treatment can permanently open, close or erase pores.

A facial may make pores appear temporarily less prominent by:

  • Removing some surface debris
  • Reducing congestion
  • Improving hydration
  • Smoothing the outer skin surface
  • Controlling excess oil as part of a wider plan

Pore appearance is also influenced by genetics, oil production, acne, skin elasticity and sun-related change.

Can Hydrafacial remove wrinkles?

Hydration and surface exfoliation may temporarily soften the appearance of very fine dehydration lines.

The treatment cannot:

  • Remove deep wrinkles
  • Relax expression muscles
  • Restore substantial facial volume
  • Lift significantly loose skin
  • Replace botulinum toxin
  • Replace fillers
  • Replace resurfacing procedures

An early small study reported improvements in fine-line appearance after a treatment course, but the evidence is not strong enough to promise wrinkle removal.

How does Dr. Cheena Langer select the treatment?

A consultation may include:

  • Identifying the patient’s main concern
  • Examining oiliness, dryness and sensitivity
  • Checking for active acne or infection
  • Looking for eczema, rosacea or dermatitis
  • Reviewing current skincare
  • Asking about retinoids and acne medicines
  • Reviewing recent peels, lasers or waxing
  • Checking pregnancy or breastfeeding status
  • Discussing expected benefits and limitations
  • Selecting or avoiding particular solutions and treatment steps

A facial may be postponed or replaced by medical treatment when an active skin disease is present.

The clinic checks for acne, sensitivity, pigmentation, rosacea, dermatitis and other conditions before selecting the facial steps.

Treatment is planned under Dr. Cheena Langer, MBBS, MD Dermatology, Consultant Dermatologist.

Dry, oily, acne-prone and sensitive skin are not given the same standard facial.

Patients with inflammatory acne, melasma, eczema or another diagnosed concern can be redirected to appropriate dermatology care.

The facial is presented as supportive cosmetic care—not as a detox, permanent pore treatment or cure for skin disease.

Hydrafacial and medifacial services are available at both Karan Nagar and Paloura.

Procedure cost depends on:

  • Facial protocol
  • Skin concern
  • Treatment duration
  • Products and solutions used
  • Whether extraction is included
  • Optional supportive steps
  • Face, neck or additional treatment area
  • Single session or treatment course

Exact pricing should be provided after assessment. A gentle hydration facial requires a different protocol from a clarifying facial for congested, acne-prone skin.

How care is planned

  • Assessment comes before treatment selection.
  • Options, limitations, aftercare and expected timelines are explained.
  • The plan is adapted to diagnosis, skin type and medical history.

Frequently asked questions

What happens during the treatment?

A customised appointment may involve:

  • Cleansing the skin
  • Examining the face under appropriate lighting
  • Performing controlled exfoliation
  • Applying a mild solution when suitable
  • Using suction-based cleansing or extraction
  • Applying selected hydrating or supportive serums
  • Using a calming mask or optional supportive treatment
  • Applying moisturiser
  • Applying sunscreen
  • Providing aftercare advice

Not every patient requires every step. Stronger or longer treatment is not automatically better.

What does the procedure feel like?

Patients may feel:

  • Coolness
  • Light suction
  • A wet or swirling sensation
  • Mild pressure
  • Gentle scratching or exfoliation
  • Temporary tingling from selected solutions

Controlled microdermabrasion is generally described as non-invasive, although temporary sensitivity can occur.

The treatment should not be advertised as completely sensation-free or painless for every patient.

Is there downtime?

Many patients resume ordinary activities after a superficial facial.

Temporary effects may include:

  • Mild redness
  • Tightness
  • Dryness
  • Temporary sensitivity
  • Mild swelling
  • Tingling
  • A short acne flare
  • Irritation from an active solution

AAD guidance for controlled microdermabrasion notes that redness or slight swelling can occur and may resemble mild windburn or sunburn for a day or two.

What complications are possible?

Possible complications include:

  • Irritation
  • Excessive redness
  • Burning
  • Bruising from suction
  • Broken superficial blood vessels
  • Acne flare
  • Contact dermatitis
  • Cold-sore reactivation
  • Post-inflammatory hyperpigmentation
  • Infection from poor hygiene
  • Aggravation of rosacea or eczema

People who tend to develop dark marks after inflammation may have a greater risk of post-procedure pigmentation.

The risk depends on skin sensitivity, products used, suction strength, technique and aftercare.

Who should postpone treatment?

A facial may need to be postponed or modified when there is:

  • Active bacterial, viral or fungal infection
  • Open wounds
  • Active cold sores
  • Sunburn
  • Significant dermatitis
  • A severe rosacea flare
  • Painful cystic acne
  • Recent facial surgery
  • A suspicious or changing skin growth
  • Recent strong peel or laser treatment
  • Severe skin-barrier damage
  • Known allergy to a planned ingredient

The AAD advises dermatologist assessment before microdermabrasion when a person has a growing or bleeding spot, scars easily or has used isotretinoin recently.

What about isotretinoin and acne medicines?

Patients should disclose:

  • Current isotretinoin use
  • Previous isotretinoin treatment
  • Prescription retinoids
  • Benzoyl peroxide
  • Salicylic acid
  • Glycolic acid
  • Antibiotic treatment
  • Other exfoliating medicines or skincare

AAD guidance warns that people taking or recently taking isotretinoin may have an increased complication risk from microdermabrasion and may need to delay treatment.

Products should only be paused when advised by the treating clinician.

Can treatment be performed during pregnancy?

A gentle facial may be possible during pregnancy, but the exact ingredients matter.

Some protocols may contain:

  • Salicylic acid
  • Retinoid-related ingredients
  • Strong exfoliating acids
  • Unverified boosters
  • Essential oils or fragrances
  • LED or device-based additions

Patients should tell the clinic that they are pregnant, breastfeeding or trying to conceive so that the treatment can be modified or postponed.

No facial should be labelled “pregnancy safe” without reviewing every product and treatment step.
